You reach sideways to put a plate away and something clicks deep inside the shoulder. Or you lift a grandchild off the floor, arm outstretched, and there is a pop followed by a brief, sharp twinge. Ask patients over 60 how the conversation usually starts and you will hear one of two lines almost every time: "it doesn't hurt, but the sound keeps happening and it bothers me," or "the pain started right around when the sound did."

The short answer is that the sound on its own is rarely a danger sign. The joint capsule holds synovial fluid, a lubricant that carries dissolved gas. Move the arm through certain angles and tiny gas bubbles can form and collapse inside that fluid, producing a click. Kawchuk and colleagues at the University of Alberta (2015) used real-time MRI to film finger joints being pulled apart and found that the moment a gas cavity forms inside the joint lines up exactly with the moment the sound occurs. It is a small study, done on finger joints in a handful of volunteers, so extending the same mechanism wholesale to the shoulder is not something the data can support on its own.

Main Causes of Shoulder Clicking

The shoulder has the widest range of motion of any joint in the body, and it pays for that mobility with relatively poor built-in stability. A large, round head of the humerus sits on a shallow socket, so tendons, ligaments, and the labrum (the cartilage rim around the socket) all have to work together to keep things in place. Let any one of them lose balance and clicking or catching tends to follow.

1. Gas release inside the joint (benign cavitation)

This is the mechanism described above: dissolved gas in the joint fluid forms a bubble that collapses and produces a sound. When there is no pain attached, clinicians generally do not treat it as meaningful. It is the same basic phenomenon as cracking a knuckle.

2. Scapulohumeral rhythm gone off track (the shoulder blade and upper arm moving out of sync)

Raising the arm properly requires the humerus and the shoulder blade to rotate together in a coordinated pattern known as scapulohumeral rhythm. When the muscles that stabilize the shoulder blade — the serratus anterior, lower trapezius, and others — weaken, or when a rounded, forward-shoulder posture sets in over years, that rhythm breaks down. The inner edge of the shoulder blade then grinds or clicks as it slides across the ribcage. Kibler's widely cited 2003 review describes this pattern, known as scapular dyskinesis, as a shared background factor behind rotator cuff injury, impingement, and several other shoulder problems. 3. Subacromial impingement (a tendon getting pinched between bones)

Between the top of the humerus and the bony ridge of the shoulder blade called the acromion runs a narrow passage carrying the rotator cuff tendons and a fluid-filled sac called the bursa. When that space narrows, or when the tendon itself is swollen, every arm raise pinches and rubs the tendon against bone, producing both a catching sensation and a click. If pain and sound both appear as you raise the arm through that same 60 to 120 degree window, this is usually the first thing worth ruling in or out.

4. Rotator cuff tear or degeneration

The rotator cuff is a group of four tendons — supraspinatus, infraspinatus, subscapularis, and teres minor — that wrap the shoulder joint. With age, these tendons commonly develop microscopic wear or partial tears that cause no pain at all. Yamamoto and colleagues (2010) scanned 664 shoulders by ultrasound in a community-based cohort in Japan and found rotator cuff tears in about 20.7 percent of them, roughly two-thirds of which were symptom-free. The rate of tears rose sharply with age. An earlier study by Milgrom and colleagues (1995), which scanned asymptomatic adult volunteers by ultrasound, found much the same pattern: tears were rare under 40 but became noticeably more common — partial and full-thickness alike — from the sixties onward. Both are cross-sectional studies, so neither one can tell us how a given tear, or the clicking that comes with it, actually progresses over time. 5. Labral tear

The labrum is the ring of cartilage that rims the shoulder socket and adds stability to an otherwise shallow joint. Repeated overhead throwing, an abrupt attempt to lift something heavy, or a fall onto an outstretched arm can tear this tissue (a SLAP tear, for instance), producing a clicking sensation at specific rotation angles along with a sense that the joint is not entirely stable.

6. Calcific tendinitis (calcium deposits building up in the tendon)

Calcium crystals accumulate inside a rotator cuff tendon, thickening and stiffening it. Louwerens and colleagues (2015) reviewed shoulder imaging from 1,219 patients and found calcium deposits significantly more often in the group with shoulder pain than in the comparison group — though a meaningful number of deposits turned up as incidental findings with no pain at all.
